Individual plans
cost ($30.00 fully refunded
) per month. Family plans cost ($30.00) for a family of five. Each of the five family members receives $10,000 coverage
all for the low price of thirty dollars per month.(FULLY REFUNDED QUARTERLY, SEMI ANNUALLY or ANNUALLY) INDIVIDUAL PLANS Individual plans are for singles (18 years
old) who want to provide this coverage for themsevles or a loved one. The plan provides $10,000 in coverage for the individual
enrolled member for $30.00 per month for the one year term. Every year each enrolled member receives a full refund on
the anniversary of their enrollment, so the cost for this coverage is "ABSOLUELY FREE". FAMILY PLANS
Family plans
provide the maximum coverage for as many as five family members all under one premium. This unique plan guarantees $10,000
for each of the five family members all for the one low monthly maintenence fee of $30.00 (FULLY REFUNDED ) Children covered
in the family plan must be 17 and under. Children covered under the family plan must enroll in the individual plan by their
18th birthday.
